WLHS Athletics Communication Policy
Open and respectful communication is essential to a positive athletic experience. Our Athletics Communication Policy outlines the appropriate steps for student-athletes and parents to address questions or concerns with coaches and staff. We encourage everyone to review the policy to ensure all communication follows the proper channels and supports a constructive team environment.

Registration
WLHS Athletics now uses FinalForms for all sports registrations.
All forms must be completed by both the parent/guardian and the student prior to the first day of practice.
In addition, a current physical must be on file in the Nurse's office (blue state physical form). Physicals should be valid through the entire season for which you are registering. All of this must be completed prior to the start of your season. Physical forms can be dropped off at the main office or faxed to the nurse's office at 860-292-5779.
If you created a FinalForms account last year, you do not need to create a new account. Log in to your current account to add a new sport and complete the required forms or update any emergency information. Remember, a parent or guardian must add the sport first before a student can review and sign their forms. A parent or guardian can also add an additional student to their account if needed.
If you are a student outside of WLPS and are looking to participate in Football, Cheerleading, or Girls Swimming, you must complete registration for your home school and for Windsor Locks by creating a FinalForms account. Be sure to select “External Student” when registering.

A PARENT/GUARDIAN MUST REGISTER A STUDENT FIRST BEFORE A STUDENT CAN CREATE THEIR ACCOUNT
NO STUDENT WILL BE ALLOWED TO PARTICIPATE UNTIL THE ONLINE REGISTRATION HAS BEEN COMPLETED BY BOTH PARENT AND STUDENT AND A CURRENT PHYSICAL IS ON FILE WITH THE NURSE.
Athlete Medical Information & Forms
The Connecticut Interscholastic Athletic Conference (CIAC) requires every athlete to have a current physical examination prior to participating in interscholastic sports. The physical is valid for 13 months from the date of the examination.
Physical forms are available in the High School main office, the High School nurse's office, or you may download the form using the link below. If your student has any health issues, such as asthma, diabetes, severe allergies, or any other that require medication, please use the link below to download the Authorization to Administer Medication form. This form must be signed by both the Physician and Parent/Guardian.
No student will be allowed to participate in WLHS Athletics without a completed Physical Form.
